PROVIDENCE, R.I. - Eyewitness News has confirmed a new surgical error at Rhode Island Hospital where a pair of forceps were discovered inside a patient's body approximately three months after the initial surgery.
According to Annemarie Beardsworth at the Rhode Island Department of Health, the patient had surgery on July 13.
On October 15, diagnostic imaging was performed for an unrelated issue. That's when doctors discovered the forceps, which were located inside the person's abdomen.
The mishap is the latest in a string of what the Health Dept. called a "troubling pattern."
Also Tuesday, the Rhode Island Department of Health fined fined Rhode Island Hospital $300,000, citing the hospital for failing to follow established policy and failing to adequately address staff reports of problems that could result in medical errors.
The fine follows an exclusive Eyewitness News story from earlier this month where we confirmed a piece of a surgical drill bit was discovered inside a patients head two days after neurosurgery.
